Instructions to Submit a Prescription Drug List to our Office
I have a scheduled appointment:
1) Submit your prescription drugs, doctors, specialists, dental, and vision providers - 5 days prior to your appointment.
2) Each individual submits a separate form.
3) If we don't receive it, we will contact you.
Re-shop my Part D Prescription Drug Plan:
1) Available Oct 1st -Nov 30th only.
2) Each individual submits a separate form.
3) We process in the order received, patiently allow us to contact you.
